SPONSORSHIP/FUNDRAISER SUBMISSION FORM

Funds for sponsorships or fundraisers will not be accepted/deposited unless accompanied by this complete and accurate form. Approved funds by check need to be made out to 'Sports Performance Youth Academy LLC' and can be put in the drop box upstairs by the main office at The Dugout or mailed to The Dugout at 4466 South Noland Rd Independence MO 64055. Approved sponsors will at minimum have their provided logo placed on the website starting January 1st. Other sponsorship opportunities, including uniforms with the sponsor's logo on the sleeve, are available by request.
